Cologuard® Instructions

A Cologuard® colon cancer screening test was ordered for you today.

Cologuard® (Exact Sciences) will contact you directly and will mail you a stool collection kit to complete.  If you do not hear from them within two weeks, please call Customer Care (844) 870-8870.

Once the test has been completed and processed, results will be sent to your healthcare provider (usually within two weeks after you submit your sample.) 

 Please check MyChart for your results; if you do not subscribe to MyChart, your provider will contact you with results.
